He is survived by: his close friend Chihuahua "Paul"; his wife Teresa Head Treadway; his daughters, Tayler Maree Treadway, Tyanna Chase Treadway and Priscilla Chapman; his father Leon Treadway; his father-in-law Kenneth Head; his grandchildren, Ryan Smith and Abel Smith; his niece Alycia Turner Anderson (Carter); his nephew Derek Turner; his son Blake; his brother-in-law Kevin Head (Brandy); his nephew Kaleb; and his niece Kynleigh Head.
