He was predeceased by: his father Jerry Douglas Cook. He is survived by: his mother Patricia Brown Cook Pyatte; his stepfather Tim Pyatte of Valdese; his brother Taylor Cook of Icard; his stepbrother Alan Pyatte (Caisey) of Hickory; his grandmother Janie Stevenson of Hickory; his stepgrandmother Carolyn Smith of Valdese; his aunt Jean Cook; and his uncle Yancy Cook (Sharon) of Hickory.
