He was predeceased by: his parents, William Clay Blackwell, Sr. and Lois Duff Blackwell; his wife Diane Blackwell; his daughters, Michelle Blackwell and Kimberly Blackwell-McKinney; and his brother William "Billy" Clay Blackwell, II.
He is survived by: his daughter Christy Cash; his siblings, Brenda Blackwell of Raleigh, Sharon Blackwell of Roxboro, Shirley Blackwell of Roxboro, Reverend David Blackwell (Elaine) of Roxboro, Steven Blackwell of Raleigh, Marcus Blackwell (Janice) of Roxboro and Hilton Blackwell of Roxboro; and his sister-in-law Zelma Blackwell of Raleigh. He is also survived by nieces, nephews, other loving relatives and friends.
