Smith (Cindy Fogle) of Winston-Salem; her sister Marylin Slater of Raleigh; her grandchildren, Holly Ward of Guerneville, CA and Phillip Gerald Smith (Jennifer) of Gerrardstown, WV; and her great grandchild Carter Gerald Smith.
