He was predeceased by: his parents, Loren Ray Slack and Jennie Dunning Slack. He is survived by: his wife Diane Smith Slack; his daughters, Kristie Catlin (Tyrece) of Thomasville and Alicia King (James) of Thomasville; and his grandsons, Elijah Hardin, Jonah Catlin, Brody Knox, Jackson King, Emmet King, Evan King, Sandy Hicks of Thomasville and Charlie Brown. He is also survived by extended family and friends.
Social distancing and mask are requested by the family.
Memorials may be given to Hospice of the Piedmont.
