He is survived by: his wife Anita Alvez-Miller (Tina); his children, Renee Mayo (Jason), Cheryl Miller, Elizabeth Norris (Lynn), Dana Alvez (Tara) and Hawkins Crisp; his grandchildren, Candice Daniels, Brandon Daniels, Kacie Manning, Adam Mayo, Tyler Mayo, Kali Norris, Audrianna Alvez and Gianna Alvez; his great grandchildren, Austin Bradshaw, Devin Bradshaw and Thomas Armstrong; his sister Debra Mansfield (Martin); and his pet Dixie.
