He was predeceased by: his parents, Halden Booth, Sr. and Hilda Chubb Booth; and his brother Halden Booth, Jr.. He is survived by: his wife Pat Mahoney Booth; his children, Brett Booth (Debbie), Tammy McClelland of Dublin, OH and Wendy Cushnie (Pete) of Dublin, OH; his sister Marlene Glick (Thomas); his siblings-in-law, Sandra Booth and Tim Mahoney (Linda); and his grandchildren, Blake, Katy and Graden.
In lieu of flowers, the family requests memorials be made to the Children's Diabetes Foundation, 4380 S. Syracuse St., Suite 430, Denver, CO 80237, or National Kidney Foundation, Inc., 30 East 33.
