He was predeceased by: his parents, Michael D'Agostino and Edna Helbig D'Agostino; and his sister Marie D'Agostino Marasuilo. He is survived by: his wife Jane Folkman D'Agostino; his children, Michael D'Agostino, Franny D'Agostino (John Merritt), Catherine D'Agostino and Emily D'Agostino; his grandchildren, Michael (Zoe), John, Justin, Charlie, Joshua, Adrien, Julia, Gianna and Brodi; and his sister Phyllis D'Agostino Carpinelli.

In lieu of flowers those wishing to make memorials are asked to consider a donation in Michael's name to St. Jude Children's Research Hospital.
