He was predeceased by: his father Millard E. Ballard; and his brother Jerry Ballard. He is survived by: his wife Ann; his sons, Destry Ray Ballard (Suzanne), Stacey Lynn Ballard and Chad Anton Ballard (Traci); and his grandchildren, Savannah, Kennedy, Destry II, Matthew and Noah Ballard.
