He was predeceased by: his parents, Milton Ray and Ila Bullock Cook; his wife Betty Cook; his grandson Bobby Longmore; and his sister Carolyn Cook.
He is survived by: his close friend Loucindy Jernigan; his children, Joannie Lane Hiott (John) of Louisburg, Michael Ray Cook (Pat) of Raleigh, Dale Allen Cook (Kathy) of Clayton and Patricia Smith Cook of Durham; his grandchildren, Christopher Longmore, Sarah Lowenstein, Kathy Cook, Ann Cook and Amy Phillips; and his siblings, Durward Cook (Peggie) of Fuquay and Violet Hammond (Ted) of Wade. He is also survived by several great grandchildren.
