He is survived by: his children, Michael Chad Hooker and Michelle Phillips (Craig); his grandchildren, Nick Hooker (Diane), Michael Hooker (Sami), Mark Hooker and Bree Phillips; and his brothers, Gene Hooker, Wayne Hooker (Brenda) and Ed Hooker (Ava). He is also survived by three great grandchildren.
