He was predeceased by: his parents, Dannie Manuel and Hettie Church Hayworth; his siblings, Martha Holman and Dorman Hayworth; and his granddaughter Hannah Hayworth.

He is survived by: his children, Brady Hayworth (Drucilla) of Purlear and Vanessa Brown (Chris) of Purlear; his grandchildren, Caleb Hayworth (Logan) of Purlear, Adam Hayworth (Kayla) of Purlear, Faith Brown of Purlear and Isaac Brown of Purlear; his great grandchildren, Cayden, Camden and Carson; and his brothers, Cecil Hayworth of Wilkesboro, Coy Hayworth of Purlear and Charlie Hayworth of Purlear.

In lieu of flowers, memorials may be made to Pleasant Grove Baptist Church Mission Fund, C/O Dorothy Greene, 248 Laurelwood Drive, Wilkesboro, NC 28697, or Yellow Hill Baptist Church Cemetery Fund, 5160 Summitt Road, Purlear, NC 28665.
