He was predeceased by: his wife Ruth Ann. He is survived by: his wife Vicki; his mother Phyllis Childers; his children, LaDonna Warren (Jeff), Heather Perry and Matthew Perry; his stepson Shawn Mulligan; and his grandchildren, Chase, Rylie and Hunter. He is also survived by three sisters and three brothers.
