He was predeceased by: his father Thomas Fagan. He is survived by: his wife Jane Daly Fagan; his children, Austin Fagan (Christine) and Maggie Fagan (Chris Rea); his siblings, Richard, William Fagan and Sheila Fagan Burnett; and his grandchildren, George Thomas Fagan, Julie Ryan Fagan, Jack Austin Fagan and Francis Dean Rea.

In lieu of flowers memorial donations may be made in Tom's memory to Shriners Hospital for Children, St. Jude's Hospital or San Juan Diego Mission Scholarship Fund.
