He was predeceased by: his parents, Herman B. Levy and Ruth Levy (Garber). He is survived by: his wife Deborah Clark Levy (Debbie); his children, Joshua Levy (Tara) of Baltimore, Maryland, Ben Levy (Stacy Landsman) of Baltimore, Maryland, Katherine Levy (Jeremy Epstein) of Baltimore, Maryland and Emily Levy (Ross Dubin) of Baltimore, Maryland; and his grandchildren, Corinne Levy, Keira Levy, Hannah Levy and Clara Epstein.
In lieu of flowers, memorial contributions may be made to UNC Lineberger Comprehensive Cancer Center in honor of the UNC Multiple Myeloma Program. Checks may be made payable to "UNC Health Foundation" with Jay Levy's name in the memo line.
