She was predeceased by: her parents, Thomas K. Smith and Hattie Leigh Adcock Smith; her husband Gale P. Lewis; her siblings, Thomas K "Smitty" Smith and Frances Bullock; and her brother-in-law Elbert Parrott.

She is survived by: her children, Gale Phil Lewis, Jr. (Kim) and Lisa R. Lewis; her siblings, David Smith (Denise), Ann Parrott, Hattie Jean Overton (Leonard) and Sally Wynne (Howard); and her brother-in-law Dale Bullock. She is also survived by nieces and nephews.

In lieu of flowers, memorials are appreciated and may be made in her honor to Providence Baptist Church, Attention: Tonya Perry, 4603 Old 75, Oxford, NC 27565.
