He was predeceased by: his father Anthony Lawrence Van Hook Sr.; and his grandparents, Anthony, Sarah Van Hook, Frank and Mary Baitinger.
He is survived by: his daughters, Rebecca "Becky" Van Hook (William "Billy" Boop) and Marianne A. Van Hook (Chris Evans); his mother Marianne Baitinger Van Hook; his grandchildren, Cayden Lawrence Lunsford, Mark Anthony Evans, Ryan James Boop and Vincent Alexander Evans; his siblings, Sarah Wansing (Kelly) and Frank Joseph Van Hook; and his close friends, Janey Hunter and Darlene Van Hook. He is also survived by numerous nieces, nephews.
In lieu of flowers, donations may be made to his daughters to assist with his final expenses.
