He was predeceased by: his mother Barbara Johnson Church. He is survived by: his sons, Joshua Shawn "Josh" Church (Loretta) of Brooklet, GA, David Shannon Church (Jammie) of Glen St and Zachary Adam Church (Haily) of Gallatin, TN; his grandchildren, Aubree, Luke, Titus and Darci; his father David Carl Church of North Wilkesboro; and his siblings, David Hayden Church (Ashley Drew) and Sharon Church Royal (David) of North Wilkesboro.
