She was predeceased by: her parents, Carl Qualls and Della Qualls; and her siblings, Sharon and Joseph. She is survived by: her husband John Forbis; her daughters, Shelly Lam (Doug), Wendy Buono (Paul) and Kelly Forbis; her grandchildren, Emily Swider (Gary), Josh Forbis (Cassidy), Josh Buono, Alex Lam, Anthony Andriole and Ben Lam; her great grandchildren, Lucas, James and Liam; her siblings, Kathy Montgomery and Scott Qualls (Becky); her aunt Bobbie Coldiron; and her cousin Ginger Hensley. She is also survived by several nieces and nephews.
