He was predeceased by: his parents, George Herman Cook and Lucy Troutman Cook; and his brothers, Conrad Cook, Allen Cook and Herman Cook.
He is survived by: his wife Mae Lee Lipe Cook; his sons, Larry Cook of Moravian Falls, Otis Lee Cook of Troutman and Terry L. Cook of Moravian Falls; his siblings, Julia Hinson of Statesville, Jerry E. Cook (Annabelle) of Stony Point, Henry D. Cook (Gaye) of Warrensville and Harold Cook of Shelby; his grandson Jason Cook of Texas; and his great grandchild Molly Cook of Texas.
