He was predeceased by: his parents, Henry Oliver McIntosh and Mabel Hager McIntosh; his wife Shirley Clodfelter McIntosh; and his brothers, Robert McIntosh and Ruben McIntosh.
He is survived by: his children, James Derwin "Jimmy" McIntosh (Laurie), Teresa Rachelle Mabardy (Paul) of Tulsa, OK and Lisa M. Yost (Mike); his brother Richard McIntosh of Statesville, NC; his granddaughters, Anna Yost Craft (Cameron), McKenzie McIntosh Crippen (Clay) and Sarah Mabardy; and his great grandchild Hudson Crippen.
