He was predeceased by: his parents, Floyd Flatt and Josephine Flatt; and his sister JoEllen. He is survived by: his wife Jackie; his children, Jeanette Avakian, David Flatt Jr., Michele Norton and Amanda Westphal; his siblings, Marianne Mellado and Tom Flatt; his grandchildren, Kristopher Hagenbuch, Ashley Norton, Shaun Gambuzza, Katie Norton, Allyson Flatt, Avery Flatt and Brandon Westphal; and his great grandchild Abigail.
