He was predeceased by: his parents, Cam Cook and Avis Morris Cook; and his siblings, Waymon Cook and Evelyn Richardson. He is survived by: his wife Violet Cook; his sons, Tony Cook of Monroe, Rusty Cook (Beth) of Monroe and Aaron Cook (Melissa) of Monroe; his sister Jane McWhorter of Monroe; his grandchildren, Emily Cook, Jordan Cook, Austin Cook (Marissa), Jackson Cook and Lela Cook; and his pet Bear.

The family requests memorials be made to a charity of the donor's choice.
