He is survived by: his wife Debra Church; his sons, Dennis Church (Sherry) of Millers Creek and Jimmy Church (Latasha) of Millers Creek; his grandsons, Jeff Ashley, Logan Church, Gunner Church and Colton Church; and his siblings, Harrold Church (Vicky) of Millers Creek and Judy Church of Shepherds Crossroads.
