He was predeceased by: his parents, William Dale and Lucy Cox Kuykendall; and his wife Helen Shelton Kuykendall. He is survived by: his stepdaughter Sandra Shelton Rollings; his siblings, Judy Baldwin, Robert Dale and Paul Kuykendall; and his stepgrandson Nicholas James Rollings. He is also survived by several nieces and nephews.

The family would like to express a special thanks to Four Seasons Hospice and in lieu of flowers donations may be made to Four Seasons Hospice, 571 South Allen Road, Flat Rock, NC 28731.
