He was predeceased by: his wife Mary Elizabeth "Betty" Mauldin Middleton; his parents, John Arthur and Naomi Jane Webb Middleton; his siblings, Vertie Loyd, Grace McCollum, Alma Fields, Mildred Poland, Inez Hendrix, Rheba Wood, Erlene Middleton, Eunice Smith, Wallace and Curtis Middleton; his grandson Jonathan; and his great grandchild Jacob "Buddy" Rice.
He is survived by: his children, Donna Thompson (Mike) and Andrew "Andy" Middleton (Dale Rea).
