He was predeceased by: his sister Paula Eugena Forte. He is survived by: his parents, Luther Forte and Shirley Forte; his wife Laura Forte; his children, Danielle Johnson (Josh), Tosha Drew (Cory), Kody Forte (Courtney) and Aulbrey Greene (Logan); his grandchildren, Arron, Eli, Malachi, Baylie, Gabe, Gannon, Milo, Briar, Alexis, Davanee, Ranger and Covan; and his sister Sharon Dula (Brian). He is also survived by numerous aunts, uncles, and cousins.
