He was predeceased by: his parent Namon Church; and his brother Lonnie Church. He is survived by: his mother Patricia Taylor Church of Sophia, NC; his wife Rita Ann Annas Church; his sons, Tony Church of Lenoir and David Church, Jr. (Amanda) of Lenoir; his brother Spencer Church (Michelle) of Sophia, NC; his grandchildren, Austin Jones, Kendra Church, Chasity Church, Toni Church, Domanic Church and Cheyenne Church; and his great grandchildren, Jamie Laney, Hunter Bumgarner, Gunner Bumgarner, Wade Church and Maggie Katosh.
