He was predeceased by: his parent Jean Kirk. He is survived by: his parent Ralph H. Ballard; his children, Rhonda Moffitt and Mike Ballard; his grandchildren, Brandy Medlin of Winston-Salem and Chris Medlin of Kernersville; his brothers, Roger Ballard, Ann Ballard of Colfax and Ralph Ballard; and his significant other Ann Keye of Winston-Salem. He is also survived by extended family and friends.
