He is survived by: his daughters, Kimberly Surber and Monya Burleson (Ryan); his grandchildren, Isabella Surber, Nadia Surber, Blaine Burleson, Breanna Burleson and Brycen Burleson; and his siblings, Betty Dennis, Brenda Levan, Randy Dennis and Janet Smith.
