He was predeceased by: his parent Herman Smith; and his brothers, Max Jr., Jack Jr. and Herman Cooke, Jr.. He is survived by: his parent Annie Overcash Cooke; his wife Carol Phillips Cooke; his sons, Nathan Buchanan Cooke and John Nicholas Cooke (Yolanda); his sisters, Phyllis Maby, Nancy Bleckley and Gayle Sebastian; and his grandchildren, Lauren Cooke (Justin Henderson), Allison Cooke, Anna Cooke, Rachel Cooke and Alexander Buchanan Cooke.
