He was predeceased by: his parents, Michael Kozak and Jeanette Kozak (LaMothe); his sisters-in-law, Barbara Piedra and Barbara Kozak; and his nephew Steve Piedra.

He is survived by: his wife Lorraine; his children, Donald, Jeff (Nicole Kozak) and Jennifer Pondish (Kozak); his grandchildren, David, Evelyn, Hudson, Meredyth and Madeline; his siblings, Michael Kozak of Arizona and Christine Kuhne (Harold Kuhne); his brother-in-law Orlando Piedra; and his pet Ginger.
