He is survived by: his wife Rachel Levan Church; his sons, Daniel Church (Crystal) of Hiddenite, NC and Mark Church (Heather) of Concord, NC; his brother Larry Church of Hudson, NC; and his granddaughters, Brooklynne Austin, Breanna, Mackenzie and Piper Church.
