He was predeceased by: his parents, Frank Polivka and Mildred Polivka; and his brother Frank Polivka. He is survived by: his wife Shirley Miller Polivka; his children, Lora Patton (Jeremy), Dawn Christianson (Kris) of Jasper and Richie Polivka; his grandchildren, Jordyn, Ciara, Stephen, Nick and Cassidy; and his brother Terry Polivka (Trish) of Naperville, Illinois.. He is also survived by numerous nieces and nephews.
