He was predeceased by: his parents, Charles Obed and Vivian Gilliam Blake; his wife Ruth Shaffner Blake; and his brothers, Eugene O'Neil Blake, Charles David Blake and Thomas Howard Blake.
He is survived by: his siblings, Dr. Jerry L. Blake (Mary Jo), Vivian Ruth Purvis, Margaret Van Poucke and Anna Blake Snider (Lee). He is also survived by his siblings and many nieces and nephews from his own family and from his wife.
