He was predeceased by: his father Alphonso Polk. He is survived by: his mother Edith Pharr Polk; his brothers, Lorenzo Polk of Kannapolis, NC and Lamont Polk of Kannapolis, NC; his cousin Precious Polk of Concord, NC; and his brother Roderick Polk of Concord, NC. He is also survived by nieces, nephews, cousins, and friends.
