He is survived by: his wife Cissie; and his children, Perry Cizek (John) of Fuquay-Varina, Steven Jenkins (Michelle Sims) of New York City and Julie Hathaway (Todd Smith) of Sturgeon Bay, Wisconsin. He is also survived by eight grandchildren and ten great-grandchildren.
