He was predeceased by: his parents, Jacquelyn Barlow Slayback and Scott Edward Slayback II; and his sisters, Pamela (George) and Debra Beare. He is survived by: his wife MaryAnne Fields; his sons, Scott Michael Slayback (Kelly) of Santa Cruz and David Thomas Slayback of Indianapolis, Indiana; his granddaughter Maranatha; his nephews, Bill Beare and Sean Fields; and his niece Erin Policastro. He is also survived by his wife and two sons.
Visitation was held on Saturday, April 26th 2025 from 11:00 AM to 12:00 PM at the Thomas Funeral Home Chapel (401 N Ennis St, Fuquay-Varina, NC 27526). A celebration of life was held on Saturday, April 26th 2025 at 12:00 PM at the same location.
