He was predeceased by: his parents, William Franklin and Nettie Dunn Blackwell; and his wife Sherri Blackwell. He is survived by: his sons, Jack Blackwell, Jr. (Tiffany) and Jordan Blackwell (Kayla); and his siblings, Billy Blackwell, Rickey Blackwell, Bruce Miller, Ray Dallas, Terry Blackwell and Kimberly Walker. He is also survived by nine grandchildren.
