He is survived by: his wife Phyllis; his parents, Rex Hartzog and Lelia Marie Hartzog; his daughters, Jessica Goldman and Kelly Parks; his grandchildren, Cameron Parks, Jillian and Jenny Goldman; and his brothers, Dennis and AJ Hartzog.
