He was predeceased by: his parents, Bryce Cook and Janie Dorsett Cook; and his brothers, Steve, Ricky and Larry Van.
He is survived by: his wife Carol Staples Cook; his sons, Tony Cook (Karen) of Troy, Wayne Cook (Tracy) of Mt Gilead, Donnie Cook of Richfield and Bryson Cook of Denton; and his brother Roger Cook (Cheryl) of Denton. He is also survived by 8 grandchildren and several great grandchildren.
