He is survived by: his parent Maude Ann Miller Polk; his wife Ada Lou Miller Polk; and his children, Charles Polk of Cleveland, Leo Polk (Lynette) of Mocksville, Carol Kelly of Winston-Salem, Kathy Hicks (Carl) of Boonville, Phyllis Knight (Terry) of Salisbury and Sandra Pennington (Mike) of Millers Creek. He is also survived by nine grandchildren and seventeen great grandchildren.
