He was predeceased by: his father David Mathis; his grandparents, Jackson Ferguson and Wanda Coward Ferguson; and his uncles, Dale, Ronald and Darrell Ferguson.
He is survived by: his children, Angela Disano (Tony), Lindsay Dale Mathis, Anna Marie Mathis McFalls (Austin) and Courtney Greene; his grandson Jackson Mathis; his mother Barbara Crawford; and his siblings, Alan Dale West, Karen Renee West Manness (Billy) and Kathy Lyn Bjerge. He is also survived by numerous aunts, uncles, extended family and friends.
Due to COVID-19, all protocols and mandates are requested to be followed, including mask.
