He is survived by: his wife Kim Kincaid Cook of China Grove; his children, Thomas Jason Cook (Norlynda) of Kingsport, TN, Jared Matthew Cook (Jennifer) of China Grove and Jenna Cook Baker (Andrew) of Kannapolis; his grandchildren, Caleb Cook, Jessyca Cook, Kinsdan Cook, Karaleigh Cook, Korlynn Cook and Presley Baker; and his siblings, Ronnie Cook and Diane Smith (Don). He is also survived by numerous nieces, nephews and other loving relatives.
