He was predeceased by: his parents, Steve Polk and Sherry Stilwell Polk. He is survived by: his fiancée Lisa Ware McIntyre; his daughters, Makayla Nicole and Laney Autumn; and his brothers, Mark Polk of Monroe, William Brian Polk (Jenae) of Gastonia and Travis Polk (Amber) of Belmont.
