He is survived by: his wife Sally Cook; his children, Jeff Cook (Gail), Bonnie (Brooks Foley) and Preston Cook; his grandsons, Joey Blackard (Ashley) and George Blackard (Gigi); and his great grandchildren, Landon, Willow, Annily, Mackenzie, Holden and Brooklyn.
