He is survived by: his daughters, Kayla Marie Carmichael and Marissa Catherine Carmichael; his parents, Patricia Faircloth Patrick and David Thomas Carmichael Sr.; and his siblings, Shannon Ferrell Page (Ryan) and David Thomas Carmichael Jr.. He is also survived by many nieces, nephews and cousins.
