He is survived by: his parents, James Edgar Hazelwood and Tammy Jean Johnson Hazelwood of Boonville; his siblings, Santana Thompson (Robert), Amanda Hazelwood and Eric Hazelwood; his grandfather Billy F. Hazelwood (Jeannie); his aunts and uncles, Kathy Spencer (Jay), Eva Johnson-Yazell (Maleah), Becky Harmon, Roy Johnson Jr., Billy Hazelwood, Jennifer Yuwapun (Boy) and Cindy Speers; and his nieces and nephews, Kobe, Kimara, Cassidy, Camryn, K'miyah, K'mora, Madison, Logan and Isabella.
