He is survived by: his wife Bernice Goodman Cook; his children, John Cook (Sandra), Nancy C. Compton (Evans), Connie C. Wiles (Clinton), Phyllis C. Bowers (Benny) and Kay C. Johnson (Cliff); his stepsons, Barry G. West and Terry B. West (Cindy); and his siblings, Herbert Cook, Elizabeth Slate and Mary Stevenson (Rick).
