He was predeceased by: his parents, Sam Ballard and Ina Schafer Ballard; his wife Patricia Anne Ballard; his children, Michael Ballard, Debra Ferry and Terry Diaz; and his sister Shirley Crowder.
He is survived by: his granddaughters, Shanise Lewis (Katherine Lindsey), Katherine Ferry and Candace Smith.
